In case you didn't know, Disability Passports collect information on reasonable adjustments through a form with details to help employers to assist an individual in the workplace to work effectively.

It's a great way of effectively communicating additional support needs within the workplace. This is useful especially if you are starting in a new role or department, as it can be incredibly difficult to "start again" and explain what you need to work or study effectively.
